I used to use somethi'n called MichaelAngelo, it was version 2.1, pretty simple and straiht forward, but that was maybe 3 or 4 years ago, you can prolly still find it.
The only annoying thing was it comes along with a bunch of other things besides icon editor/creator. like ani cursors and stuff.
I found it pretty useful for just makin' icons/cursors but there's prolly better stuff out there i didn't know of.
